Rates
$100 per 50 minute session

Insurance
As a service to you I have an insurance manager who is glad to check your insurance coverage for you. At our first session I will get a copy of your insurance card so that we can obtain authorization and benefits. If you find that the coverage is limited, please consult with me about possible payment arrangements so that you may still be seen. If you prefer, you may give us insurance information in advance so that we can check insurance coverage or you can check the coverage yourself by asking the following questions of your insurance company:

• Do I have mental health benefits?
• What is my deductible and has it been met?
• How many sessions per calendar year does my plan cover?
• How much does my plan cover for an out-of-network provider?
• What is the coverage amount per therapy session?
• Is approval required from my primary care physician
• Am I responsible for a co-payment for each session?
• Because I have a co-insurance policy, what is the percentage I am responsible for?

Reduced Fee
Reduced fee services are available based on personal need to be determined at initial counseling session.

Payment
Cash, check and all major credit cards accepted for payment.

Cancellation Policy
If you do not show up for your scheduled appointment, and you have not notified me at least 24 hours in advance, you may be required to pay the full cost of the session.
